    *Each individual player must fill out a Medical/Media Waiver prior to the start of games.  You can download a copy in our Club Documents section or pick up a copy at any of our registration events.

    TEAM REGISTRATION FEE FOR CURRENT USLA PLAYERS IS $15.00 PER PLAYER

    3 ROSTER MINIMUM, 6 PLAYER MAXIMUM: $45 AND $15 FOR EACH ADDITIONAL PLAYER

    TEAM REGISTRATION FEE FOR NON-USLA TEAMS IS $30.00 PER PLAYER
    (SI USTED NO JUGAR CON USLA EL CUOTA DE INSCRIPCIÓN DEL JUGADOR ES $30.00)
    3 ROSTER MINIMUM, 8 PLAYER MAXIMUM: $90 AND $15 FOR EACH ADDITIONAL PLAYER

    ACCEPTANCE OF TERMS & CONDITIONS

    In consideration of the acceptance of my membership in Urban Soccer Leadership Academy of San Antonio, I, the participant and parent/guardian, agree as follows that once I electronically submit this form & payment:

    En consideración de la aceptación de mi membresía en Urban Soccer Academia de Liderazgo de San Antonio , yo, el participante y el padre / tutor , de acuerdo como se desprende que una vez que me presento electrónicamente este formulario y pago: 

    1. I understand that I or my child cannot play in any sanctioned soccer game until after this registration form and medical release has been validated and the registration data has been entered into the registration system. / (Entiendo que yo o mi hijo no puedo jugar en cualquier partido de fútbol sancionado hasta después de este formulario de inscripción y autorización médica ha sido validado y los datos de registro se ha entrado en el sistema de registro)
    2. I have reviewed the waiver/participation agreement attached and my signature affixed hereto indicates my agreement with such waiver/participation agreement. / (He revisado el acuerdo de exención / participación adjunta al presente y mi firma puesta indica que estoy de acuerdo con dicho acuerdo de exención / participación.)
    3. I am aware of USLA bylaws, policies, rules and regulations and agree to abide by them and to be bound by them. / (Soy consciente de los estatutos USLA, políticas , normas y reglamentos y estoy de acuerdo en cumplir con ellos y estar obligado por ellos.)
    4. I accept sole responsibility for my or my child’s personal possessions and athletic equipment. / (Acepto la responsabilidad exclusiva de las posesiones personales de mi o de mi hijo y el equipo atlético.)
    5. I acknowledge that I have read this registration agreement in its entirety and that I have executed this registration agreement voluntarily.
    6. I understand that after the application is submitted; all Fees are NON-REFUNDABLE.

    ***USLA IS responsible for Player Game Day Tee, Referees for U10 & U12 Games Only, & Field Expenses

    ***USLA IS NOT responsible for meals, player cleats, shin guards or personal soccer ball.
          USLA no es responsable de las comidas, cornamusas jugador, espinilleras o balón de fútbol personal.
