    *Each individual player must fill out a Medical/Media Waiver prior to the start of practice.  You can download a copy in our Club Documents section or pick up a copy at any of our registration events.

    PERSONAL INFORMATION
    For Parents/Guardians ONLY:
    All parents/guardians are required to volunteer either for a Leadership Council position -OR-  a minimum of four (4) events/games for one hour (4 hours total each season). 
    Please select a box for either the Leadership Council Position Options OR Minimum Hours Options.
    ***(Team Coach Player Registration Fee and Fundraising waived for one (1) player each season when parent volunteers for a Leadership Council Position.  For a complete description of each position, please refer to the parent handout)
     ***(Sign up hours may be done online through the "SignUpGenius" link at USLAsoccer.org as listed on the parent handout.  Sign up times are on a first come first serve basis)
    NOTE:  Failure to complete the selected volunteer commitment option will result in a $25.00 penalty and ineligible player status.

    PER PLAYER COST, FUNDRAISER, AND SCHOLARSHIP DISCOUNTS

    Fundraiser and Scholarship Discounts are given as a way to offset the Operational Costs incurred each time a player registers to play a programmed season with USLA.  Each season an Annual Budget will be available for review and approval by the Board of Directors.  If you select the Fundraising Discount Options, you are required to participate and complete the fundraising commitment(s) by the deadlines listed in the Master Schedule.

    Recaudador de fondos y becas Descuentos se dan como una manera de compensar los costos operativos incurridos cada vez que un jugador se registra para jugar una temporada programado con USLA . Cada temporada un presupuesto anual estará disponible para su revisión y aprobación por parte del Consejo de Administración. Si selecciona las opciones de descuento de recaudación de fondos , usted está obligado a participar y completar el compromiso de recaudación de fondos dentro de los plazos que figuran en la Lista Maestra.

    ACCEPTANCE OF TERMS & CONDITIONS

    In consideration of the acceptance of my membership in Urban Soccer Leadership Academy of San Antonio, I, the participant and parent/guardian, agree as follows that once I electronically submit this form & payment:

    En consideración de la aceptación de mi membresía en Urban Soccer Academia de Liderazgo de San Antonio , yo, el participante y el padre / tutor , de acuerdo como se desprende que una vez que me presento electrónicamente este formulario y pago: 

    1. I understand that I or my child cannot play in any sanctioned soccer game until after this registration form and medical release has been validated and the registration data has been entered into the registration system. / (Entiendo que yo o mi hijo no puedo jugar en cualquier partido de fútbol sancionado hasta después de este formulario de inscripción y autorización médica ha sido validado y los datos de registro se ha entrado en el sistema de registro)
    2. I have reviewed the waiver/participation agreement attached and my signature affixed hereto indicates my agreement with such waiver/participation agreement. / (He revisado el acuerdo de exención / participación adjunta al presente y mi firma puesta indica que estoy de acuerdo con dicho acuerdo de exención / participación.)
    3. I am aware of USLA bylaws, policies, rules and regulations and agree to abide by them and to be bound by them. / (Soy consciente de los estatutos USLA, políticas , normas y reglamentos y estoy de acuerdo en cumplir con ellos y estar obligado por ellos.)
    4. I accept sole responsibility for my or my child’s personal possessions and athletic equipment. / (Acepto la responsabilidad exclusiva de las posesiones personales de mi o de mi hijo y el equipo atlético.)
    5. I acknowledge that I have read this registration agreement in its entirety and that I have executed this registration agreement voluntarily. / (Reconozco que he leído este acuerdo de registro en su totalidad y que he firmado este acuerdo de registro voluntariamente).
    6. I understand that after the application is submitted; all Fees are NON-REFUNDABLE. / (Entiendo que después de presentar la solicitud; Todas las tarifas no son reembolsables).
    NOTE:  Failure to complete fundraisers will result in player ineligibility AND a ban from USLA for all future seasons.
    NOTA: Si no se completa la recaudación de fondos se traducirá en el reproductor de inelegibilidad y una prohibición de USLA para todas las estaciones futuras.

    ***USLA IS responsible for Player Insurance, Game day Security, Uniforms, Referees,  & Field Expenses
          USLA es responsable de Seguros Player, Seguridad el dia de los juegos, Uniformes, árbitros, y campo Gastos

    ***USLA IS NOT responsible for meals, player cleats, shin guards or personal soccer ball.  Each Team must designate their own Coach & Team Manager
          USLA no es responsable de las comidas, cornamusas jugador, espinilleras o balón de fútbol personal. Cada equipo debe designar su propio entrenador y jefe de equipo.
